Getrag failure - what to do?

My uncle has a 91 1 ton dodge (work truck) that recently has had trouble getting stuck inbetween or in gear. They have to pull the shifter out and manually using a bar or rod get things lined back up to get it to work again. It has quite a few miles on it, not sure how many though.

What would be the best thing to do?

Rebuild Getrag transmission?? How much money and are these tranny's decent enought to justifiy rebuilding it?

Swap in a NV4500 if possible? What is needed to do this? Direct drop in replacement or very involved?
