W250 front brakes

How are our front brakes set up? Are the rotors separate from the hubs or are they one unit? Mine are well worn from the low pad indicator and the guy at NAPA didn't know which one to sell or quote me.

1992 W250 auto, dually conversion, 2" Skyjacker front lift, ASA/AFE intake, H1C/60 -16, Bosch 190's, Bully Dog fuel pin, 366 spring, modified pump, timing advanced and a 4" turbo back exhaust. Suncoast valve body plumbing 3.54's w/LSD. Autometer gauges.
