power steering

I have a '92 4x4 with 323K miles. The power steering is going. When it is cold there is no power steering, when it warms up it's better, but slow and makes that whine that I have heard from vehicles with bad power steering. It has started to leak a little fluid, but has not gone below the min mark. How can I tell if it is the steering unit or the pump. Or should I just replace both. How hard is it to do? Do I need special tools? Are reman units good or do I need to find new ones? Where? I love this truck, bought it new, want to keep it going.
