rear disc brake conversion

I have a RAM 2500 that I have made into a one ton. I had a DANA 70 under it and now I have a DANA 80 from a chasis cab dually. My question is, can I convert from drum brakes to disc? What needs to be done to convert this thing? The new trucks have disc brakes on the rear so I assume that I can convert mine. Any suggestions?
