Need Help Identifying Problem.....

I have been experiencing some weird things with my truck in the past month. It is an 04.5 600 with a Quadzilla Chip and AFE Intake. There are 34,000 miles on the truck.

1. I was going to make a left turn and was speeding up a little to make the light. The RPM's began to surge but stayed between 2100-2400RPM's. It was almost like I was revving the motor up. I pulled over safely, turned off the truck, and then started it back up and it hasn't happend since.

2. I was coming down a hill and the PacBrake was engaged and I was about to turn and the truck stalled. There was no shuddering, no nothing. It just shut off. Again, rolled to a safe stop and turned the key and the truck started right up again. Hasn't happened since.

3. This morning as I was headed up the GSP, I noticed a lot of smoke coming from the tailpipe when I accelerated. I was running about 70-75MPH and the RPM's were in between 2000-2200 RPM's. When I accelerated, the truck let out a white smoke with a hint of black. At one point, the cloud was almost all black. Also, when the smoke would come out, the engine appeared to be choking as if there was not enough fuel going in. But, after about 10 miles of seeing the smoke, it stopped and now the truck runs normal.

The only thing I could think of was either bad fuel or an injector sticking in event 3. But, could the lift pump be going and these are just some indications? I have plugged the truck in the past week or so because temps have gone down to 30 at night. Should I take the truck and hook a scanner up to it and see what happens before I take it to the dealer? I have no idea what could be wrong, so thanks to anyone who can help me with this.
