Blown turbo #4 on an 05

This is on my son's truck. An 05 auto with a TST Powermax CR. 25 k miles
Dealer has been waranteeing them. One blew up before he got it home from the dealer...10 miles.
This one has lasted a couple of months.
The TST is set to defuel at 39 psi. He hasn't towed or hauled anything with the last two turbos. He does have a very heavy foot but his juiced up 94 still survives.
Is this normal for an 05 to chew up stock turbos with a TST box?
