'03 stays in high idle

A friend of mine has an '03 w/ a Cummins HO, stock, eliminated muffler, and 90,000 miles. He says that he has a really hard time w/ it sticking on high idle. Like when he pulls up to a stop light, it will stay revved on high idle. He says the only way he can get it to quit, is to lug the engine way down. He took it to the dealer and had the computer scanned. It showed no trouble codes. So they replaced his lift pump off the engine, to the in-tank model. Still the problem exists. Also, he says he will occasionally have a rough idle. What would cause this?
