Cummins survey, free cab plates

A month or so back, someone posted a link to the Cummins site and if you filled out the survey on the Cummins/Dodge Ram, they would send you a cab plate. Did anyone ever get the cab plate? I sent an e-mail to Cummins recently, thinking that my name and address had fallen into the crack someplace. They replied that they were unaware of the survey and asked who was supposed to supply the cab plate.? I know my memory is slipping, but I am pretty sure that I recall seeing the survey on the Cummins site and did fill it out. Does anyone recall this survey and perhaps have the link?
