Nv4500 Question

I was wondering if the nv4500 has a mechanicaly or hydraulic actuated clutch throw outbearing or clutch fork. and what kind of price can i expect to pay for a good unit, and where can i find one. This is going into a 95 suburban with a second gen 12v infront of it. thanks in advance for your help, I'm glad there is such a wealth of info on this page.
