need new 5spd probably

227,000 miles ran low on oil, changed oil, leaking severly, added castrol synthetic, so now, after my cranial/rectal surgery, what are my options? Transmission is grumbling even in neurtal and grinding between a couple of gears, and noisy in all gears. So, what can I do....new dealer transmission? nah, junkyard? rebuild the one in there? rebuilt one? What years do I consider? Is there a clutch upgrade to consider? I plan on keeping truck forever. The truck is a 1996 2500 4wd ext. cab. Thanks in advance, I live in the middle of NC. Steve
