drastic change in fuel mileage

My wife drives a stock 98 2500. The fuel gauge doesn't work so she fills up based on mileage. She usually fills up at about 300 miles. Last week she ran out of fuel TWICE. She had 200 miles on the clock. What's happening.
Overflow valve? I'm not going to bring her fuel again. If it happens again, I'll have a slightly used wife in the classifieds.
