Something whining?

I have a 1999 Dually, 3500 24Valve and am experiencing a whining noise that speeds up with the engine R.P.M..I've got 187,000 miles on it and have replaced the belt tentioner, and had the alternator rebuilt. The mechanic that rebuilt the alternator checked the water pump and the idler pulley and said that they both seemed to be fine. Has anybody else experienced this problem? and if so what will fix it? This whine will almost cover up the whistle of the turbo. I'd appreciate some suggestions.
