07 5.9 has died...Help!

I was having some problems getting the truck to turn over, it would after 6-7 tries, rough idle as well. I decided to take her to the dealer and motor started knocking as I drove down the freeway and Bam! Knocking and A LOT of smoke...engine dead. I have run a few tanks of ASTM certified B100 but has always run fine and I know a lot of other contractors that use this fuel/station and all is well for them. Any ideas? Injector shot?

No word from the dealer yet. Damn.
