Seriously LOW Fuel mileage and smokin

Mine is a 98 3500 12v 4x4. I recently purchased it and it is getting about 10mpg!!! Smoking an awful lot too. Is their anyone here in the eastern PA /Phila. area who works on these or can recommend a shop to help me perhaps take car of what is a timing issue. I have a trip coming up end of aug. to Michigan, would like to get it remedied before then.
